  You can initiate an evaluation for your patient’s orthotic or prosthetic care by faxing a prescription/referral for evaluation to any of our 5 office locations. We do not charge for evaluations.

• Step 2:

  We will obtain your patient’s history and arrange the free evaluation for care.

• Step 3:

  We will provide you with ample clinical documentation, all procedure codes, quantities, and contracted rates for the necessary care.

• Step 4:

  Our formal referral should list all procedure codes and quantities. Westcoast Brace & Limb’s TAX ID is 592066601. Please call us, should you have any questions.

• Step 5:

  We provide your patient with high quality orthotic or prosthetic care and share the outcome with you.
